image-only spam -- ideas, what to do?

you should have a look at spamitarium or stripsearch (its in the contrib
directory of the bogofilter tarball.) those scripts can add/remove more
information to the mails and make it easier for bogofilter decide if a
mail is ham or spam. I've found stripsearch to be quite good without
mangling things too much (at least for my liking).

though it doesnt directly solve the problem, i find most inlined images
have a link to somewhere and stripsearch just works for mails like that.
