Rob,

The problem is the capital letters.  Prior to 0.14.x bogofilter only 
recognized "Subject:" when looking to insert the spam_subject_tag.  As of 
0.14.x the comparison uses strncasecmp() to avoid case 
sensitivity.  Unfortunately, strncasecmp() isn't doing what I 
expected.  I'll have to dig deeper...
