It looks as though the docbook XSL stuff could not currently handle a
programlisting or screen within an orderedlist properly for man output,
while HTML or PDF are fine (go figure, try
xmlto html-nochunks bogoupgrade.xml).

I filed SourceForge Bug #628393 against the docbook project.
