All of these were used at one time.  As they may be needed again and
unused rules have no effect on the generated code they've been left in
the file in case they're needed at some future time.  They could be
moved into comments, but there's no need for so doing.

BASE64 and QP are referenced in the 09/01/03 comment.  TTYPE was removed
early yesterday (about noon GMT).
