t.lock2 working

Hi,

I have somewhat radical changes in my bogofilter work directory that
make it pass t.lock2.

It will need some refinements and fixing bogoutil that got broken by my
swirling cleanup axe that hit database_db.c pretty hard. bogoutil knows
DB internals it is not supposed to know (violation of module scope) --
database_db.h is going away, database.h is getting an iterator to allow
access to all data.

This is still not the final state (transactions), and I may need to
close and reopen a data base for big registration jobs to maintain
scalability.

So there is finally progress on the locking front.
